Lower Eyelid Blepharoplasty

Lower eyelid blepharoplasty is a commonly performed procedure in facial aesthetics, aimed at addressing concerns such as excess skin, prominent fat pads, and under-eye hollowing—including tear trough deformities and deep infraorbital grooves.

Given the delicate and complex nature of the lower eyelid, careful attention to anatomical detail is essential to avoid complications and ensure optimal results. A thorough evaluation of the patient’s anatomy allows for a tailored treatment plan that is both safe and effective.

Different surgical approaches may be selected depending on the patient’s specific needs. In cases where there is no excess skin, a transconjunctival approach—accessing the eyelid from the inside—can be used to modify the fat pads. This may involve conservative reduction or repositioning of the fat to smooth the transition between the lower eyelid and cheek, effectively improving tear trough deformities. In selected cases, fat grafting can be combined with this technique to further enhance volume and contour.

When excess skin is present, the procedure can be combined with a pinch blepharoplasty technique, as performed by Dr. Mirza Fırat, to precisely remove the necessary amount of skin without compromising eyelid function.

If lower eyelid support is insufficient, a canthopexy may be added to reinforce the eyelid and reduce the risk of complications such as eyelid retraction, which can be challenging to correct.

Lower eyelid surgery is also frequently combined with Dr. Mirza Fırat’s endoscopic midface lift. This approach provides structural support to the lower eyelid, enhances overall facial harmony, and further minimizes the risk of complications. In such cases, endoscopic canthopexy may also be performed to ensure optimal stabilization of the lower eyelid.
