REGARDING THE USE OF THE RIGHTS GRANTED TO PERSONAL DATA SUBJECTS
PURSUANT TO THE LAW NO. 6698 ON PROTECTION OF PERSONAL DATA
Your requests regarding your personal data processed by Op. Dr. Mirza Fırat as a data controller in our clinic located at Zorlu Center Teras Evler No:008 Beşiktaş, must be submitted to the above-mentioned address by hand or by registered mail, after filling out and signing this form completely and clearly.
Our company, as the data controller, will respond to your request in writing or electronically, as soon as possible and within thirty days at the latest, free of charge or, if the transaction requires an additional cost, in return for the fee to be charged according to the tariff determined by the Board.
In the event that our company accepts the request, the necessary action will be taken, and if our company is defective in the request of the respective person regarding the implementation of the Law, the fee will be returned to the respective person.
REQUESTS OF THE PERSONAL DATA SUBJECT
The rights you can request from our company, which is the data controller within the scope of the LPPD, under Article 11 of the Law are listed below:
a) Learning whether any of your personal data has been processed;
b) Requesting information regarding the processing activities of your personal data;
c) Learning the purposes of processing your personal data;
d) Learning these persons if personal data has been transferred to third persons at home or abroad;
e) Requesting for correction of the personal data if they are processed incompletely or inaccurately;
f) Requesting the deletion or destruction of personal data in case the reasons requiring the processing of personal data disappear or the Company does not have a legal basis or legitimate interest to process the said data;
g) Requesting the Company to ensure that third persons, which are also authorized by the Company and process personal data, respect your rights under this section;
h) Objecting to adverse results that may arise as a result of processing personal data through automated systems; and
i) If you suffer damage due to the unlawful processing of your personal data, you have the right to demand compensation for this damage.

3 – INFORMATION TEXT
The data such as name & surname, address, identity number, e-mail address, signature and telephone number data of the applicant in this application form is automatically collected and processed for the purposes of preventing data breaches and informing the respective person by Op. Dr. Mirza Fırat, pursuant to Article 5 of the Law No. 6698 on Protection of Personal Data, with the legal reasons "it is clearly stipulated in the law" and "it is mandatory for the data controller to fulfill its legal obligation". Your personal data that you have forwarded for the purpose of exercising your right of application are not transferred to other third persons at home or abroad.
4 – APPLICANT'S DECLARATION
I request that all my requests stated in this application I have made to Op. Dr. Mirza Fırat company be considered in accordance with Article 13 of the LPPD and informed to me. For this purpose, I hereby agree, declare and undertake that the documents and information I have given to you in the application are correct, up-to-date and belong to me.
